We have two ways for you to get a card NOW.  Choose what is best for you:


1.  Click here to immediately download or print an 8.5" x 11" card with a pharmacy listing & FAQ.  You must hand write your information on the card prior to using.

2.  Click here to personalize and print a card.  We will email a link to you.  Follow the link, input your information and your card will be personalized for you to print.
