Instructions for Use:

Go to the eye doctor for an eyeglasses exam. There are several possibilities to help you with the cost of an eye exam.

If you are 65 or over and have not been to an eye doctor in at least three years, you may qualify for a free exam by an ophthalmologist. Click here for details.

If you are an uninsured worker, you may qualify for a free or reduced cost exam through one of the 17 Optometry Schools and Colleges located throughout the country. Click here for a listing.

Obtain your prescription. It MUST have the following information in order to process your order:

  • OD-AXIS (Right):
  • OD-CYL (Right):
  • OD-SPH (Right):
  • OS-AXIS (Left):
  • OS-CYL (Left):
  • OS-SPH (Left):
  • PD-Pupillary Distance:

Order your glasses. You have two options:
